Psychiatric Rehabilitation Program Counselor

Job Description

The PRP Worker is responsible for the direct provision of rehabilitation and support services to participating consumers. PRP Workers visit consumers in their homes and communities, and provide services in the home, community, or program site.

ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING
  • Conduct regularly scheduled consumer visits, (6) for adults and (3) for minors monthly.
  • Provide identified PRP rehabilitation and support services in accordance with the consumer's Individual Rehabilitation Plan (IRP).
  • Conduct Rehabilitation groups and activities to address rehabilitation and support service needs.
  • Maintain required and consistent documentation for each client, including contact/progress notes, monthly summaries, and quarterly reviews.
  • Participate in ongoing trainings and supervisions.
  • Maintain a manageable caseload to ensure outcomes and practice requirements are met while taking into consideration the level of care of clients served.
  • Have their own vehicle, that is insured.
  • Perform all duties within contract, state and federal regulations, Compassionate Care Services, LLC policy, and standards.
  • Writing, interpersonal, and oral communication skills
  • Knowledge of the DSM and relevant symptoms of the most common diagnosis within the PRP programs.
  • Other duties may be assigned as necessary.
  • Demonstrate an understanding and sensitivity to serving culturally diverse and/or special needs population.
Qualifications for Mental Health Therapist
  • 1-2 years’ experience note writing skills
  • Must submit to and pass a pre-employment drug test.
  • Must successfully pass all background screening required by the state.
  • Completed CPR/First Aid training
  • Possess a valid driver’s license in good standing with three (3) or less points.
  • Reliable transportation and auto insurance that complies with the minimum state requirements
  • Ability to travel locally.
  • Attend mandatory meetings as required by the agency.
  • Help clients solve problems, identify resources and make effective use of resources
  • Candidate must be computer literate and demonstrate strong organization, time management and writing skills.
  • Understands and maintains confidentiality and all regulations regarding HIPAA
  • Exhibits professionalism and a friendly demeanor, and is a positive representative of the company over the phone and in person
  • Solid written and verbal communication skills.
  • Comfort with computers and technology as needed to input clinical documentation.
  • Strong organizational skills to manage multiple clients and scheduling.
  • DLA-20 Certification
  • Reliable, ethical, and honest work ethic.
  • Job Types: Full time, part-time, contract.

  • Education, Licensure and/or Experience: An Associate's Degree or equivalent college credits from a four-year college, in social work, psychology, mental health or another related human services field. At least one (1) year of previous experience working in mental health field, preferably as a PRP worker.

  • Salary: $18.00/h - $30.00/h experience based
